Information we collect. In order to operate the web-based payment services, ScheduleMD must ask you to provide us information about yourself and your checking account and/or credit card account. The information we request may include, but not limited to your name, address, email address, driver's license number, government-issued photo-ID number or loyalty card account information and checking account/credit card information.
Updating Account information. ScheduleMD will not modify your information without your permission, except in the case of correcting an obvious error (such as your zip code). You are responsible for keeping your financial account information with the enrolled bank account up to date. You may update or correct your information at www.schedulemd.com
Information we may disclose. ScheduleMD may disclose information on transactions that you make to our participating merchants as permitted or required by law, including companies that perform marketing services for us or with whom we have joint marketing agreements. These agreements allow us to extend the value of your membership or provide you with special savings. Information will normally be limited to gender, zip code and/or medication(s) taken. No personal information will be disseminated that will allow a patient to be identified.
For the purpose of this Agreement and consistent with the HIPAA Privacy Rule, “Minimum Necessary” is defined as that protected health information that is “reasonably necessary to achieve the purpose of the disclosure” and is disclosed to only “Those persons or classes of persons, as appropriate, in its workforce who need access to protected health information to carry out their duties.”
Consistent with the HIPAA Privacy Rule, in no case will the limited data set include any of the following identifiers:
1. Names
2. Postal address information (other than town or city, state and zip code)
3. Telephone numbers
4. Fax numbers
5. E-mail addresses
6. Social security numbers
7. Medical record numbers
8. Health plan beneficiary numbers
9. Account numbers
10. Certificate/license numbers
11. Vehicle identifiers & serial numbers, including license plate numbers
12. Device identifiers & serial numbers
13. Web Universal Resource Locators (URL’s)
14. Internet Protocol (IP) address numbers
15. Biometric identifiers, including finger and voice prints
16. Full face photographic images and any comparable images
Assurances
1. Recipient shall not use or further disclose the limited data set other than as permitted by this Agreement or as otherwise required by law.
2. Recipient shall use appropriate safeguards to prevent use or disclosure of the limited data set other than as permitted by this Agreement.
3. Recipient shall ensure that any specified agents, including a subcontractor, to whom it provides the limited data set agrees to the same restrictions and conditions that apply to the limited data set Recipient with respect to such information.
4. Recipient shall not re-identify the information or contact the individuals for whose records are contained within the limited data set.
Third parties
At ScheduleMD, maintaining your trust and confidence is our top priority. We do not sell or rent non-public personal information about our members or former members. We do not share non-public personal information about our members or former members with non affiliated third parties except where required or permitted by law. We carefully manage and safeguard information among our companies and affiliates.
Security. We restrict access to your non-public information to those employees or affiliates who need such information in order to provide products, customer or collection services. In compliance with federal regulations, we maintain physical, electronic and procedural safeguards designed to guard and prevent misuse of your non-public personal information.
Notification of Changes. ScheduleMD may revise its privacy policy from time to time. If we are going to use or disclose your personally identifiable information in a manner materially different from that stated at the time we collected the information, you will have a choice as to whether or not we use or disclose your information in this new manner. Any material changes will be effective only after we provide you by e-mail with at least 30 days notice of the amended Privacy Policy. (If you have cancelled your membership, you will not be contacted to notify you of the amended Policy and your personal information will not be used or disclosed in this new manner). We will post the amended Privacy Policy prominently on our website so that you are aware of what information we collect, how we use it and under what circumstances, if any, we may disclose it.
Customer Service Correspondence. If you send us correspondence including e-mails and faxes, we retain such information in the records of your account. We will also retain customer service correspondence and other correspondence from ScheduleMD to you. We retain these records in order to measure and improve our customer service and to investigate potential fraud and violations. We may, over time, delete these records if permitted by law.
